Privacy Agreement 

I certify that the facts set forth in this Application for Employment (Application) are true and complete to the best of my knowledge.  I understand that if I am employed, false/incomplete statements or information on my Application, or during my application process, may result in my dismissal.  I hereby authorize U.S. Cellular to investigate any of the facts set forth in this Application which may include, but is not limited to, a credit check or criminal records background check.

 

I understand that employment with U.S. Cellular is "at will", which means that either I or the Company can terminate the employment relationship at any time, with or without prior notice, and for any reason not prohibited by law.  All employment is continued on that basis.  I understand that no supervisor, manager or executive for the Company, other than the president, has any authority to alter the foregoing and then only if it is in writing.  I hereby authorize U.S. Cellular, its employees or agents, to contact the references as well as current or previous employers listed on this Application (unless I have not authorized contact with my current employer in the online application).  I also authorize the person or organization listed on the Application to give information concerning my previous employment, education or any other information which they may have, personal or otherwise, with regard to any of the subjects covered by the U.S. Cellular Application for Employment.  I hereby release all such parties from liability for furnishing such information.

 

I understand U.S. Cellular is a drug-free workplace, and I understand an offer of employment is contingent upon successful completion of U.S. Cellular's drug screening.  Any positive test will negate an offer of employment.
